FSMS Clinic
Welcome to the FSMS clinic website. The school clinic is available to service both the students and staff. The program is designed to assure a safe, healthy environment that is conducive to learning and to provide professional care for those who become ill or injured while at school.
Parents and teachers are encouraged to contact the nurse and/or assistant of any health conditions or problems affecting your students. We can better serve the students to promote or improve his/her health and/or reduce lost time from the classroom with this input.

School nursing is a specialized practice of professional nursing that advances the well-being, academic success, and life-long achievement of students. To that end, school nurses facilitate positive student responses to normal development; promote health and safety; intervene with actual and potential health problems; provide case management services; and actively collaborate with others to build student and family capacity for adaptation, self-management, self advocacy, and learning.

Important information regarding immunizations!
Attention parents of current 6th Graders!
Students entering the 7th grade in the upcoming school year are required by the state of Texas to show proof of having 3 vaccines prior to the start of the new school year. These vaccines are:
· Tdap (tetanus, diphtheria, and acellular pertussis) – vaccine must have been given within the last 5 years.
· Varicella (chickenpox) – 2 doses of the vaccine are required unless your child has had the actual disease…which must be verified by the child’s physician.
· Meningicoccal (Meningitis) – 1 dose of the vaccine is required.
Information for all immunization requirements can be found at The Texas Department of State Health Services’ website http://www.dshs.state.tx.us/immunize/public.shtm. In many instances, your child may have already received these vaccines and we simply do not have an updated record. Please submit those records as soon as possible.
